The Community Connector Program targets individuals from culturally and linguistically diverse backgrounds with disabilities who require support to access the NDIS or who require additional support with NDIS or other services. This service is free.
What Community Connectors can do:
- Help build better community awareness of the NDIS
- Assist individuals, their families and/or carers to complete an NDIS Access Request Form
- Assist individuals, their families, carers and representatives to attend assessments and collect medical evidence that will support their NDIS application
- Attend NDIS pre-planning, planning, and review meetings if clients would like our help
- Connect people with NDIS service providers
- Assist disability service providers to better engage with people from CALD backgrounds to NDIS services
You are eligible…If you are…
- Aged 7 to 64 years with a permanent disability
- From a Culturally and Linguistically Divers (CALD) background
- Have a Permanent Residency visa or you are an Australian citizen living in Melbourne metro eastern local government areas.
